Backup Request Form

Use this form to request that a new machine be added to the nightly backup schedule. If accepted, the machine will backed up to the ECF data storage system.

All fields are required. It's okay to use the same person for owner, primary user, or technical contact, if appropriate. To submit more than one machine, simply fill out and submit the form multiple times. If you use your browser's "back" button to return to this page, it should keep the fields filled in each time.

Hostname:
This should be the fully-qualified hostname of the machine to back up, e.g. "granite.essc.psu.edu".

Owner:
The individual who actually owns the machine, and should receive the bills for backup and archive services.
